COVID-19 Safe Patron Contact Form

In order to keep Centennial Parklands Sports Centre a Covid-19 Safe venue and to adhere to the NSW Governments COVID-19 Safety Plan, we are now required to keep a record of every patron who has attended the facility.

This information will be kept private for a period of 28 days and only shared with the necessary organisations for tracking COVID-19.
